Currently, he is still alive and fighting alongside the Rebels. He fought in the Siege of Mandalore, and helped Ahsoka Tano escape when his men turned on them.

The Rebels series finale, which aired on March 5, 2018, confirmed that Rex did fight at the Battle of Endor, but did not go so far as to confirm that Rex and Sant are the same character. Now he didn't die in the Clone Wars because the creators wanted to connect an old Captain Rex with this old man spotted during the Battle of Endor (Return of the Jedi). During the execution of Order 66, Captain Rex escaped with Ahsoka Tano, where they parted ways.
